The facility's oversight was compounded by a lack of recent audits of resident medical records to ensure all necessary documentation was present.

The facility failed to maintain electrical equipment in a safe condition due to the use of an extension cord in the dining room. Specifically, a lamp was found plugged into an extension cord, which was then plugged into a surge protector and a wall socket.

The facility lacked a consistent system for tracking returned medications to the pharmacy and was not properly documenting the specific dates of medication administration on pill packs.

The facility failed to maintain complete resident records, specifically lacking the necessary documentation such as the FL-2, Resident Register, Care Plan, and TB test results within the sign-in book during respite stays.
