The facility failed to ensure that exit doors were equipped with a sounding device to alert staff when opened for a resident known to be a wanderer. Specifically, two out of three sampled exit doors lacked an active alarm. This was observed during a site visit where the front wooden door was not closed and the front glass door alarms failed to activate.

The facility failed to ensure that 2 out of 3 sampled staff members were tested for tuberculosis (TB) disease upon employment. Personnel files for Staff B and Staff C lacked documentation of the required TB skin tests at the time of their hire.
