Mills Estate Villa

The inspection was an unannounced annual visit conducted on February 4, 2026. The primary deficiency found relates to the improper handling and storage of resident medications. Specifically, medications were found removed from their original containers, which was cited as an immediate risk to resident safety.

The unannounced case management visit was conducted to follow up on elevator repairs. The administrator provided a tour, and the LPA observed necessary safety measures, including evacuation chairs and alternative transportation arrangements. No deficiencies were cited during this inspection.

The inspection identified multiple critical deficiencies across several areas, including facility cleanliness, medication storage, and safety protocols. Specifically, there were citations regarding grease stains in the kitchen, expired food items, unsecured medications, and missing required signage for oxygen use. All cited deficiencies were classified as Type A, indicating an immediate health, safety, or personal rights risk.

The unannounced annual inspection on February 14, 2024, found the facility generally compliant in areas such as safety hazards, room amenities, and medication/chemical storage. However, one deficiency was cited regarding the failure to keep all residents with Dementia current on their Medical Assessments (LIC 602).
