An on-site compliance inspection was conducted on December 3, 2025, and the following deficiency was cited:

Based on observation and interview, the manager failed to ensure that medication was stored in a separate, locked, self-contained unit. The deficient practice posed a risk to residents who were not prescribed the accessible medication. Findings include: 1. During an environmental inspection, the Compliance Officer observed R2's Morphine medication stored in an unlocked box in the refrigerator located in the facility's kitchen. 2. In an exit interview, E2 acknowledged that the medication box was unlocked at the time of the survey.
