An on-site compliance inspection and investigation of complaint 00107068 was conducted on March 5, 2026, and the following deficiency was cited:

Based on observation and interview, the manager failed to ensure that medication was stored in a locked or self-contained unit. The deficient practice posed a risk to residents who were not prescribed the accessible medication. Findings include: 1. During an environmental inspection, the Compliance Officer observed R3's Insulin Pen medication stored in an unlocked box in the refrigerator located in the facility's kitchen. 2. In an exit interview, the findings were discussed with E1, and no additional information was provided.

Based on this deficiency-free compliance inspection, the Department shall not conduct a compliance inspection for twenty-four months, according to A.R.S. \'a7 36-425(E).
