Based on record review and interview, the manager failed to ensure a resident had a written service plan reviewed and updated no later than 14 calendar days after a significant change in the resident's physical, cognitive, or functional condition, for one of two residents sampled. Findings include: 1. A review of R1's medical record revealed a hospice order for wound care (dated July 12, 2023). 2. A review of R1's medical record revealed a service plan for personal care services (dated in June 2023). However, the service plan did not include hospice wound care. 3. In an interview, E1 acknowledged a service plan, updated within 14 calendar days after R1's admission to hospice. was not available for review.
