Livingston Post Acute Care receives high praise for its dedicated nursing and rehabilitation staff, with many families highlighting individual caregivers who provide compassionate, personalized support. However, critical reviews point to significant issues with understaffing, slow response times to call bells, and concerns regarding medication safety and patient monitoring. While many patients report successful recovery, the facility's consistency in care quality appears to fluctuate depending on the shift and staffing levels.

Key Review Excerpts

The facility is beautiful but it is not for someone who is going in for rehabilitation and has other health care needs. The experience I had with my father for just one week was absolutely ridiculous. First off there is NOT enough staffing her to handle the high acuity of patients.

Rehab patient's family · 2025☆☆☆☆

The best feature is the physical and occupational therapy staff. I got therapy usually every day. Yolanda was one of physical therapy staff who encouraged me with a sense of humor. It receives two stars because the aides on the morning and afternoon shifts go on vacation and NEVER answered call bells during their last hour.

Rehab patient · 2025★★☆☆☆

Upon discharged once we got home they gave him another patients medication; a womens at that. My sister took it back & they gave her the right medication. It seems like nobody took this issue serious.

8 complaint-triggered

This facility shows concerning patterns with families filing complaints that triggered inspections revealing issues with nursing staff levels, resident assessments, pain management, and discharge planning. The most recurring problems involve resident assessment and care planning, medication management, and safety/fire code violations across multiple surveys from 2021 to 2024. While most deficiencies appear corrected, the consistent issues with core care areas and multiple family complaints suggest ongoing challenges with quality oversight and care coordination.
