The facility failed to maintain a matching therapeutic diet menu for physician-ordered diets to guide food service staff. Specifically, three sampled residents with orders for mechanical soft, chopped meats, and diabetic carbohydrate-controlled diets did not have corresponding menu guidance available.

The facility failed to ensure each resident had the opportunity to participate in at least one outing every other month. Observations and interviews revealed that residents had not been on an outing in approximately eight months due to limited transportation and a lack of planned activities. Additionally, residents had not been consulted regarding their preferences for activities and outings.

Observations revealed trash, dirt, and unidentified particles on floors in multiple rooms, as well as separated floor tiles and black substances near toilets and sinks. Interviews indicated the Medication Aide/Supervisor was responsible for cleaning but lacked a formal cleaning schedule and had not recently deep cleaned along baseboards.
