The facility failed to maintain a matching therapeutic diet menu for physician-ordered diets to guide food service staff. Specifically, three sampled residents with orders for mechanical soft, chopped meats, and diabetic carbohydrate-controlled diets did not have corresponding menu guidance available.

The facility failed to ensure each resident had the opportunity to participate in at least one outing every other month. Interviews and observations revealed that residents had not been on an outing in approximately eight months due to limited transportation. Additionally, residents reported that they were not being asked about their preferences regarding activities and outings.

Observations revealed trash, dirt, and unidentified particles on floors in multiple rooms, as well as separated floor tiles and black substances near toilets and sinks. Interviews indicated the Medication Aide/Supervisor was responsible for cleaning but lacked a formal cleaning schedule and had not recently deep cleaned along baseboards.

The facility failed to provide a non-disposable place setting consisting of at least a knife, fork, spoon, plate, and beverage containers during meal service. Observation of the lunch meal service revealed that the required table service components were not being met.

The facility failed to ensure that a proper criminal background check was completed prior to hire for three staff members. The documentation found in the personnel records consisted only of local county searches rather than the required statewide record searches.
