What this means for your family

While some families report positive experiences with specific staff members and therapy, there is a concerning pattern of reports regarding poor hygiene and inadequate medical oversight. We strongly recommend visiting the facility in person at different times of the day, specifically checking the cleanliness of resident rooms and asking for a detailed plan on how they manage dietary needs for diabetic patients.

This facility shows a concerning pattern with 36 deficiencies across multiple surveys, with recurring issues in fire safety, food service, and care planning persisting across all three inspection periods from 2021 to 2025. The most troubling aspects include repeated problems with food safety and storage, fire safety equipment and procedures, and developing proper resident care plans. While all violations have correction dates, the persistence of similar issues across years suggests ongoing operational challenges that families should carefully evaluate before considering placement.
