What this means for your family

While the rehabilitation therapy department is frequently cited as a strength, the facility has a consistent pattern of severe care failures regarding basic needs like toileting, medication, and call light response. Families should exercise extreme caution and perform frequent, unannounced visits to ensure their loved one's safety, as the gap between professional feedback and family experiences is significant.

Lakeview Post Acute receives highly polarized feedback, with a significant number of families reporting severe neglect, including unaddressed call lights, poor hygiene, and medication errors. While some reviewers, particularly those associated with the facility or recent visitors, praise the therapy team and recent management changes, the recurring reports of patient abandonment and unsanitary conditions are deeply concerning.

Key Review Excerpts

The bright spots at Lakeview are the therapists (PT, OT, and speech) as well as Devin Warren and Mirissa Weber. Other than these people, there is little to recommend at this facility.

Rehab patient's family · 2025★★☆☆☆

I will say that the therapy team was great! And I really appreciate what they tried to do for me. While some of the staff seemed to genuinely care, most did not. I had several times of waiting anywhere between 3-5 hours to be put back in bed.

Rehab patient · 2024☆☆☆☆

My father was here for five days and was neglected. They did not give him his medicine. No oxygen. Not sure he was even fed. He fell multiple times.
