What this means for your family

This facility has a strong rehabilitation program and many dedicated staff members who provide compassionate care. However, because multiple families have reported serious issues with hygiene and communication, we strongly advise you to conduct unannounced visits during weekends or evenings to verify the standard of care for yourself.

Lake Montgomery Health and Rehabilitation Center receives highly polarized feedback, with many families praising the dedicated therapy team and individual staff members for their kindness. However, significant concerns regarding basic hygiene, understaffing, and poor communication persist, with multiple reports of residents being left in soiled conditions or families being ignored during medical emergencies. Prospective families should weigh the positive experiences of rehab patients against these recurring reports of neglect and administrative unresponsiveness.

Key Review Excerpts

My 85yr old dad came here to go through physical therapy after getting sick and going into the hospital. All the Administration, Physical Therapy, Nursing,CMA and all the rest of the staff were wonderful.

Rehab patient's family · 2026★★★★★

I would not recommend this facility for someone who is incompetent. I came in to visit my grandfather on several occasions and he was fully soaked in his own urine. I never got updates and would always have to find someone to ask about his progress.

Long-term resident's family · 2025★★☆☆☆

My mother has been sitting in a wet diaper for over 6 hours. This isn't the first time, she's told me this. Maybe I should send some inspectors in to see what's really going on in this facility.

9deficiencies
Above state avg (7.3)
3 complaint-triggered

Families filed complaints leading to three recent deficiencies in January 2026 involving failure to notify families of resident changes, inadequate care planning, and medical record issues. The facility shows recurring problems with resident assessments and care planning across multiple surveys, along with ongoing fire safety concerns including sprinkler system maintenance. While most historical issues have correction dates, the recent complaint-triggered deficiencies suggest ongoing concerns with communication and care coordination that families should investigate during visits.
