Specifically, live and dead bed bugs were observed in a resident's bedroom, and live and dead roaches were observed on the dining room floor. Additionally, the facility had not requested necessary pest treatment from their contracted provider.

The facility failed to ensure that 1 of 2 sampled staff members had documentation of a tuberculosis (TB) skin test completed upon hire. While the staff member reported receiving tests, there was no documentation of the results or a hire date in the personnel record.
