The facility failed to maintain a clean and orderly environment free of hazards. Specifically, live and dead bed bugs were observed in a resident's bedroom, and live and dead roaches were observed on the dining room floor. Additionally, the facility had not requested necessary pest treatment from their contracted provider.

The facility failed to ensure that 1 of 2 sampled staff members had documentation of a tuberculosis (TB) skin test completed upon hire. While the staff member reported receiving tests, there was no documentation of the results or a hire date in the personnel record.

The facility failed to maintain a clean and orderly environment free of hazards. Inspections revealed live bedbug activity and black debris on a resident's bed, as well as live cockroach activity and debris on the floor in another resident's room.

Specific observations included live bedbugs, debris on mattresses, and thick dust on furniture. Additionally, resident bed linens were not changed following the discovery of pests, and pest control treatments had not been updated for over a year.

The facility failed to maintain mattress covers free from bedbug residue in resident rooms #3 and #4. Observations revealed brownish smeared stains resembling bedbug residue on the mattress covers, and staff were unaware when the covers were last changed.

The facility failed to protect the kitchen area from contamination related to live roaches. During the inspection, live roaches were observed crawling on the kitchen walls and floor near the stove, and paper plates containing roach powder were found near the stove and trash can.
