The facility failed to ensure that two exit doors had a sounding device that was engaged, continuous, and audible when opened. This was noted for residents who were intermittently disoriented. Specifically, the main entrance and the kitchen door alarms sounded upon opening/closing but did not provide a continuous sound.

The facility failed to ensure that two of its two exit doors had a continuously sounding device that was engaged, continuous, and audible when opened. This was a failure to protect two residents who were identified as being intermittently disoriented.

The facility failed to follow proper procedures for medications prepared in advance. Specifically, the facility did not ensure that medications were properly identified, protected from contamination, or stored in labeled, sealed containers as required by the regulation.

The facility failed to ensure a medication aide observed residents taking their medications. Observations and interviews revealed that medication containers with resident initials were left on the dining room table, and residents reported that their medications were often placed in cups at their seats prior to their arrival for breakfast.

The facility failed to provide non-disposable place settings during meal service. Specifically, four residents were observed being served iced tea in disposable red plastic cups during lunch.

The facility failed to serve water to residents during the noon meal. Observation revealed that no water was offered to the four residents present, and the facility's cycle menu did not list water as part of any meal services.
