A licensure revisit was completed on 7/16/25 for all previous deficiencies cited on 12/31/24. The regulations governing Assisted Living Residences were revised. The new Chapter VII regulations were implemented on 3/17/25. Based on interview and record reviews, the residence failed to ensure personnel files for current employees were onsite and readily available for department review for one sample staff (#6), affecting seven current residents. This deficiency was cited previously during a state licensure survey on 12/31/24. Although the facility corrected the deficiency, based on the findings below, the facility has not maintained compliance with this regulatory requirement.Findings include:1. Record ReviewOn 7/16/25 at 9:23 a.m., the personnel file for Staff #6 was requested and not provided.On 7/16/25 at 9:56 a.m., a staff list was provided, which revealed Staff #6 was hired on 4/9/25. The June and July 2025 staff schedule revealed that Staff #6 worked from 7:00 a.m. to 9:00 p.m. on 6/6, 6/7, 6/13, 6/14, 6/20, 6/21, 6/27, 6/28, 7/4, 7/5, 7/11 and 7/12/25.2. InterviewOn 7/16/25 at 11:39 a.m., the administrator designee stated Staff #6' s personnel file was at the administrator' s house. The administrator designee further stated she was unaware of the requirement for personnel files to be onsite, and was unsure why the deficiency had not been corrected.
