Insufficient staffing levels: The facility did not always have sufficient staff on duty for the supervision, personal care, and protection of residents. Specifically, shifts with only 1 staff member were found to be inadequate given that 6 of 8 residents required assistance with ambulation and 7 of 8 required total assistance with personal hygiene/toileting. Additionally, reports indicated times during the midnight shift when no staff was present.
