Questions for Your Tour

  • 1I noticed that the facility has a very modern, hotel-like atmosphere; how do you ensure that this high standard of care extends to timely responses when a resident uses their call light?
  • 2Given the facility's strong reputation for physical and occupational therapy, how do you coordinate those sessions with daily medication management to ensure my loved one stays on track?
  • 3I see that the leadership team is very active in responding to feedback online; what is your preferred process for keeping families updated on their loved one's daily progress and health status?
  • 4Since the facility has a high volume of residents, what specific protocols are in place to ensure consistent care for those who may have higher cognitive or memory-related needs?
  • 5Could you walk me through how your nursing staff handles medical emergencies or sudden changes in a resident's condition during the evening and weekend hours?
  • 6What kind of social activities or community events are available to keep residents engaged and active outside of their scheduled therapy sessions?

Key Review Excerpts

The nurses left her in bed and didn't change her out of her dirty clothes. They take forever to answer call lights. She has several wounds from neglect.

Memory care family member · 2025☆☆☆☆

My dad caught pneumonia, and had to be rushed to the ER bc he could not pee. Catheter was clogged with a blood clot. Numerous times I came to visit and and my father had an accident and was left to sit in it.

This facility has concerning patterns with multiple families filing complaints that led to citations, including a recent serious complaint about protection from abuse and neglect. The facility shows recurring problems with resident rights, care planning, and safety systems across multiple surveys, with issues like emergency lighting, fire safety equipment, and nursing staffing appearing repeatedly. While the facility has provided correction dates for deficiencies, the persistence of similar problems across years suggests ongoing challenges with consistent quality care and safety compliance.
