What this means for your family

This facility is an excellent choice for short-term rehabilitation, particularly for those needing intensive physical therapy. However, families of patients with memory issues or those requiring high levels of constant supervision should be cautious, as staffing levels may not be sufficient for those specific needs.

Questions for Your Tour

  • 1I noticed your team is very active in responding to online feedback; how do you use that family input to improve daily operations and communication?
  • 2Given the high praise for your physical and occupational therapy programs, how do you integrate those services into a resident's daily routine?
  • 3With the current staffing ratings, what specific protocols do you have in place to ensure call lights are answered promptly and residents receive timely assistance?
  • 4Since communication is a top priority for our family, what is the standard process for keeping us updated on changes in care or medical status?
  • 5How does the facility approach memory care to ensure residents feel engaged and supported, given the specialized nature of that programming?
  • 6What is your protocol for handling urgent medical needs or emergencies, and how quickly would we be notified if a situation arises?

Key Review Excerpts

My 90 year old grandmother worked her tail off in therapy there for 7 weeks and is now off of oxygen, eating better, walking with a walker, and doing almost all of her own ADLs now!!

Granddaughter of resident · 2022★★★★★

In my opinion, if your loved one has dementia or memory issues, this is NOT the place for them. I personally had to provide about 75 percent of my dad's care.

Daughter of resident · 2024☆☆☆☆

My mother was here for three days. In that time they ignored her, her room was kept at freezing temperatures, take over 30 minutes to get anyone to answer her call light and would take over an hour to get pain medication.

1 complaint-triggered

This facility shows a mixed record with 12 deficiencies across multiple surveys, including one complaint-triggered violation where families reported concerns about failure to notify them of resident injuries or health changes. The most recurring issues involve fire safety systems, medication management, and resident care planning. While most deficiencies have been corrected, a recent fire safety violation from January 2025 remains without a correction plan, which is concerning for resident safety.
