An initial licensure survey was completed on 11/14/24. A deficiency was cited. Based on observation and interview, the residence failed to be in compliance with all applicable regulations.This failure created the potential for mismanagement of the care of and services for the residents who would be served by this residence.Findings include:-21.2On 11/14/24 at 9:45 a.m., during an exterior environmental tour of the residence revealed slopes and hazards as follows:The residence' s backyard had a patio that connected with the grass and had a drop-off ledge of more than six inches, which posed a fall risk for residents who used this area.On 11/14/24 at 9:49 a.m., the administrator stated she was aware of the requirement for the exterior environment to be free of slopes and hazards; however, she had placed chairs where the steepest drop-off ledges were. The administrator acknowledged that all drop offs around the whole patio parameter (including the steepest drop-offs) created a potential hazard. -22.21On 11/14/24 at 9:10 a.m.., an environmental tour of the upstairs common area bathroom revealed shower flooring that was smooth with no evidence of safety features to prevent slips and falls. On 11/14/24 at 11:09 a.m., the administrator stated it had been a couple of years since the shower pan (shower flooring) in the upstairs common bathroom had been installed and was unable to find evidence to show the shower pan was slip-resistant. The administrator acknowledged she was aware of the requirement for the shower pan to have safety features to prevent falls.
