Specifically, staff did not consistently use well-fitting face masks to reduce the risk of COVID-19 transmission and infection during resident care encounters.

Staff members were observed answering the front door, performing resident screenings, and transporting residents in the facility vehicle without wearing face masks. Additionally, staff were observed walking throughout the facility and performing tasks without masks while residents were present.

The facility failed to ensure blood pressure checks were implemented and documented as ordered for a resident with a diagnosis of hypertension. Specifically, there were no entries for daily blood pressure readings or documentation of refusals in the medication administration record for the months of February, March, and April 2021.

The facility failed to ensure that one sampled staff member had a completed criminal background check in their personnel record. The Administrator had instructed the staff member to obtain the check but had not verified the report before allowing the employee to begin work.

The facility failed to obtain pharmaceutical care services from a licensed pharmacist, prescribing practitioner, or registered nurse at least quarterly for three sampled residents. This lack of oversight prevents the identification and resolution of medication-related problems.
