Observations revealed heavy black mold on refrigerator door seals, improper storage of eggs above ready-to-eat foods, and excessive frost buildup in freezers with broken door seals. Additionally, freezer temperatures were recorded at 18 and 20 degrees, which is above the recommended FDA standard of 0 degrees or below.

The facility failed to ensure kitchen and food storage areas were clean, orderly, and protected from contamination. Observations revealed black mold on refrigerator door seals, heavy frost/ice buildup in freezers, and improper food storage with eggs placed above ready-to-eat items like chicken salad and lettuce. Additionally, a freezer thermometer recorded an unsafe temperature of 20 degrees.

The facility failed to maintain walls, floors, ceilings, or fixtures in a clean and repaired manner. Specific issues included a broken vinyl floor tile and peeling wallpaper in resident room #8, as well as an improperly fitted window A/C unit that created gaps and drafts.
