The facility failed to ensure a resident with a physician-ordered pureed diet and honey-thick liquids was served the correct consistency. Specifically, a resident was served regular milk and cereal instead of pureed food, resulting in coughing and labored breathing. Additionally, staff lacked proper training on preparing therapeutic diets as ordered.
