The facility failed to ensure that one of three sampled staff members was tested for tuberculosis disease in compliance with required control measures. Specifically, there was no documentation of a TB skin test being completed upon the staff member's hire at the current facility.

During lunch service, 19 residents were observed with only a spoon provided, lacking forks and knives. Interviews revealed residents were sometimes given only plastic spoons and noted that knives were rarely provided.
