A relicensure survey with complaint #CO32632 was completed on 3/14/24. Deficiencies were cited. Based on interview and record review, the residence failed to ensure each qualified medication administration person (QMAP) accurately documented each medication administration at the time the event was completed for each resident, affecting two of three sample residents (#2, #3). Residence Policy The residence' s Medication Administration policy, dated 1/2020, read in part: "Each QMAP, nurse, or practitioner must accurately document each medication administration or monitoring event at the time the event is .. Based on observation and interview, the residence failed to ensure all medications were stored in a locked storage area when unattended by a qualified medication administration person (QMAP) or other licensed staff, affecting all current residents. Findings include: On 3/13/24 at 8:00 a.m., and environmental tour revealed the medication storage area was located on the main level of the residence across from a common are restroom and Resident #2' s bedroom. The medication storage area contained a locked cabinet with medications the residents were currently taking. Howe.. A certification complaint, prompted by #CO32634 was completed on 3/14/24. A deficiency was cited. Based on observation, interview, and record review, the facility (residence) failed to follow written policies and procedures for the administration of medication in accordance with 6 CCR 1011-1, Chapter VII, affecting three of three sample participants (residents) (#1-#3).Findings include:1. Chapter VII regulations governing assisted living residences, part 14.21, requires residences to comply with authorized practitioner orders associated with medication administration except for those medications which a resident self-administers.Reference and Residence PolicyThe residence' s Medication Administration policy, dated 1/2020, read in part: "The assisted living residence is responsible for complying with authorized practitioner orders associated with medication administration except for those medications which a resident self-administers ..." According to MedlinePlus, "Quetiapine tablets and extended-release tablets are also used alone or with other medications to treat episodes of mania (frenzied, abnormally excited or irritated mood) ... If you suddenly stop taking quetiapine, you may experience withdrawal symptoms such as nausea, vomiting, and difficulty falling asleep or staying asleep ..."a. Resident #1 was admitted to the residence on 4/3/23 with a diagnosis of dementia. Quetiapine A written practitioner' s order, dated 7/25/20, directed the residence to administer quetiapine 25 mg two tablets at bedtime (HS). However, the May 2023 medication administration record (MAR) read from 5/8-5/18/23, the medication was unavailable. A total of 11 doses were missed.On 3/14/24 at 3:15 p.m., the care coordinator (CC) stated she was aware that Resident #1 had been out of her quetiapine between 5/8–5/18/24 because the pharmacy stated there was an issue with the insurance provider and they would not pay for it.
