Provider failed to conduct risk assessments required by Governor's Safe Start Plan and DOH for community outings (medical appointments, employment, shopping).
Client 1 (seatbelt) and Client 2 (hoyer lift/bed rails) lacked required written instructions and/or consents for device use.
Monthly bank reconciliations were missing verification signatures.
Clients 1, 2, and 4 did not have signed consents for locked thermostat and chemicals secured with latching straps.
90-day nursing visits for Clients 1 and 2 were not current (last visits 7/10/21).
Clients 2 and 3 had outdated Release of Information forms.
Client 1: Blood pressure readings not recorded 7 times. Client 2: MAR lacked specific time for PRN laxative; referral to nutritionist not followed up.
Hole in drywall with exposed wires in bathroom; Client 3 lacked documentation of evacuation drills/water temperature checks.
Client 1 wheelchair seatbelt (restrictive procedure) lacked ETP approval by Administrator or DDA.
