The facility failed to ensure that two sampled staff members, including the Administrator, had completed the required state-approved medication aide training courses. Additionally, one staff member had not completed the clinical skills validation checklist or passed the state written medication aide exam.

The facility failed to ensure that one of two sampled residents was tested for tuberculosis disease upon admission in compliance with public health control measures. Specifically, a review of Resident #2's records showed no evidence of a first or second step tuberculosis test being completed.

The facility failed to ensure contact with the resident's physician for verification or clarification of medication orders. Specifically, for Resident #1, several medications including Omeprazole, Albuterol, Zofran ODT, and Imodium were being administered but were not included on the resident's current FL-2 physician orders.
