Critical concerns include inconsistent weekend staffing, poor communication, and inadequate nutrition, which contrast sharply with positive reports of a caring, professional team. Prospective families should be aware of these significant discrepancies in the quality of care provided.”

Inspection History
Medicare Inspection History
3-year lookback · Medicare 2026
Heartland Rehabilitation has a pattern of recurring fire safety and smoke detection violations across multiple surveys, with the same deficiencies reappearing in 2024 and 2025 despite corrections. The facility also shows ongoing issues with food safety and resident care quality, including problems with nutrition services and assistance with daily living activities. One family filed a complaint about pressure ulcer care in 2023, though all deficiencies show correction dates from the provider.
