Key Review Excerpts

I will NEVER recommend this facility to anyone. They neglected my mom who was there a short time on hospice with TERMINAL BRAIN CANCER, including but not limited to withholding meds, administering another person RX to her, never bathed her,cut off all of her hair, never changed her clothes or bed linens, misplaced her multiple times.

Hospice patient's family · 2024☆☆☆☆

My mom went there for 5 days so I could get a much needed respite and they were all so kind to her. They kept me updated via cellphone and made everything so much easier for her and me.

Respite care family member · 2023★★★★★

The weekday shift is great but the weekend shift need some work. I know when the cats away the mice will play.

1deficiencies
Well below state avg (4.3)

Heartland Rehabilitation has a pattern of recurring fire safety and smoke detection violations across multiple surveys, with the same deficiencies reappearing in 2024 and 2025 despite corrections. The facility also shows ongoing issues with food safety and resident care quality, including problems with nutrition services and assistance with daily living activities. One family filed a complaint about pressure ulcer care in 2023, though all deficiencies show correction dates from the provider.
