A recertification survey was completed on 10/2/24. A deficiency was cited. Based on observation and interview, the facility failed to ensure the members' right to privacy and right to be free of cameras in the interior areas of residential settings including common areas and residential hallways, affecting three current members (#1-#3).Findings include:On 10/2/24 at 7:40 a.m., an environmental tour was conducted, during which all doors that went from inside to outside the facility had chimes to alert when the door was opened.On 10/2/24 at 11:29 a.m., the administrator stated she was unaware that the facility could not have chimes to alert when the doors were opened and closed.

A relicensure survey was completed on 10/2/24. Deficiencies were cited. Based on record review and interview, the residence failed to have policies and procedures to ensure the continuation of care to all residents for 72 hours following an emergency including, but not limited to, a long-term power failure, affecting six current residents.On 10/2/24 at approximately 8:15 a.m., a 72 hour continuation of care policy and procedure was requested but was not provided.On 10/2/24 at 11:00 a.m., the administrator stated she was aware of the requirement to have a written policy and procedure for the 72 hours following an emergency. The administrator confirmed she did not have a 72 hour continuation of care policy and procedure and thought all the required elements were in the emergency plan. Based on record review and interview, the residence failed to have readily available a roster of current residents along with a residence diagram showing room locations, and the emergency contacts for each resident, affecting five current residents.Findings include:On 10/2/24 at approximately 8:15 a.m., the residence' s resident roster for emergency preparedness was requested.On 10/2/24 at 9:10 a.m., the residence' s resident roster was provided. However, the resident roster did not include a diagram of the residence that showed room locations.On 10/2/24 at 1:25 p.m., the administrator stated she was unaware of the requirement for the resident roster to have a diagram of the residence and be readily available. THIS PORTION OF THE REPORT IS FOR INFORMATIONAL PURPOSES ONLY.No response is necessary.The residence was advised it must review and maintain the following processes in accordance with existing program regulations found at 6 CCR 1011-1, Chapter 7.14.11 Only medication that has been ordered by an authorized practitioner shall be prepared for or administered to residents.14.21 The assisted living residence shall be responsible for complying with authorized practitioner orders associated with medication administration except for those medications which a resident self-administers.14.31 The administrator and the QMAP supervisor shall, on a quarterly basis, audit the accuracy and completeness of the medication administration records, controlled substance list, medication error reports, and medication disposal records. Any irregularities shall be investigated and resolved. The results of the audits shall be documented and routinely included as part of the assisted living residence' s Quality Management Program assessment..
