A relicensure survey was completed on 8/13/25. Deficiencies were cited. Based on interview and record review, the residence failed to ensure the administrator completed the additional 10 hours of training as required, affecting seven current residents. Findings include: On 8/12/25 at 8:00 a.m., the 40-hour administrator training certification was requested. However, the administrator training certification provided, dated May 2016, contained only 30 hours of the required training. The administrator was unable to provide the additional 10-hour training required. On 8/12/25 at 1:41 p.m., the administrator stated she was aware of the require.. Based on observation and interview, the residence failed to assist residents who requested access to activities outside of the residence, affecting three sample residents (#1-#3). Findings include: On 8/12/25, the undated monthly activities schedule was posted on the wall; however, it did not include any scheduled activities outside of the residence. On 8/12/25, at 8:05 a.m., Resident #1 stated there were "no activities" available at the residence, and that he would like to be able to "help others" and walk around.On 8/12/25, at approximately 10:10 a.m., when asked wha.. The section of the deck where residents accessed the outdoor patio furniture was warped in some areas. A recertification survey was completed on 8/13/25. Deficiencies were cited. Based on interview and record review, the facility (residence) failed to ensure a lease, residency agreement, or other written agreement was updated annually for each member (resident), affecting three of three sample residents (#1-#3).Findings include: The records for Residents #1-#3 revealed the agreements were signed on 10/10/23, 1/5/24, and 9/6/23, respectively. On 8/12/25 at 1:49 p.m., the administrator stated she was unaware of the requirement that the lease/residency agreements should have been updated annually. She stated that the only time an agreement would have been updated was if a resident transferred to a different room or went from a semi-private to a private room. Based on record review and interview, the facility (residence) failed to thoroughly document the required information when modifying member rights for one sample member (resident) (#1). Findings include:1. Record ReviewResident #1 was admitted to the facility on 9/6/23 with a diagnosis of mild dementia. A practitioner ' s order, dated 7/8/25, read in part, "(Resident #1) was restricted to walking only on the facility premises due to safety." A care plan for Resident #1, dated 6/29/25, read in part, "(Resident #1) was permitted to be unsupervised solely in his room."The record for Resident #1 failed to contain a modification of the member' s rights. 2. InterviewsOn 8/12/25, at 8:05 a.m., Resident #1 stated there were "no activities" at the facility, and he would like to be able to walk around and "help others."On 8/12/25, at 8:20 a.m., the administrator stated she does not want Resident #1 walking past the facility grounds.On 8/12/25, at approximately 10:10 a.m., Resident #1 stated the administrator and staff prohibited him from leaving the facility to walk around the neighborhood due to a recent fall. Resident #1 went on to state that the administrator "is a control freak; it' s her way or the highway." He said, "Before I fell, I was only able to be gone for forty-five minutes from the facility."
