The facility failed to ensure that 2 of 3 sampled staff members had completed required medication aide training, clinical skills validation, and the written examination within the required timeframes. Specifically, one staff member lacked documentation of completing the 5-hour training prior to administering medications and did not pass the written exam within 60 days of completing the clinical skills checklist.

The facility failed to ensure that 2 of 3 sampled staff members (Staff A and B) completed medication aide training, clinical skills validation, and the written examination within the required timeframes. Specifically, there was no documentation of 5-hour training for both staff members, and both failed to pass the written exam within 60 days of completing clinical skills validation.

The facility failed to ensure that residents' medical examinations were updated annually. Based on record reviews, the facility failed to maintain annual updates for 2 of 3 sampled residents (#1 and #3).

The facility failed to ensure that one of three sampled residents had an assessment and care plan updated annually. Specifically, Resident #2 had a care plan dated 11/15/21 that had not been updated despite the resident being seen by a physician in March 2023.

The facility failed to ensure medication administration records were accurate for 2 of 3 sampled residents regarding oxygen use. Specifically, Resident #1's eMAR lacked entries for continuous oxygen at 3L despite an active order, and the resident was observed using a portable tank set at an incorrect level of 6L.

The facility failed to ensure medication administration records were accurate for 2 of 3 sampled residents regarding oxygen use. The facility failed to maintain proper medication cart security. As part of the corrective action, the medication cart will be subject to random monitoring by Medical Assistants and the Administrator, with daily checks for one month followed by weekly ongoing monitoring.

The facility failed to ensure staff documented the administration of medications immediately following administration. During observations, a medication aide administered inhalers and nasal sprays to residents without updating the medication administration records (MARs) before moving to the next resident's medication.

The facility failed to maintain clean and well-repaired floors, walls, and surfaces. Specific issues included thick dust and dirt buildup on baseboards, air vents, and behind furniture, as well as trash accumulation in common living areas and resident bedrooms. Additionally, the facility failed to maintain the floor surface in the men's common bathroom where strips of material were raised and uneven.
