The investigation was conducted in response to multiple allegations, including issues with incontinence care, food services, supervision, supplies, and facility repair. Based on interviews and observations, the Licensing Program Analyst found that there was not a preponderance of evidence to prove any alleged violations occurred.

The facility underwent an unannounced annual inspection on October 21, 2025. The Licensing Program Analyst reviewed resident and staff files, and toured all areas including apartments, bathrooms, common areas, kitchen, laundry, medication room, and courtyard. No health or safety violations were observed during the inspection.
