Key Review Excerpts

During this time they gave her a catheter as they didn’t have enough staff to take my Mom to her restroom. The catheter wasn’t cared for, my Mom developed a horrible UTI and when we asked the nurse on duty to please check because we saw that something was very wrong she said she would after she finished

Rehab patient's family · 2024☆☆☆☆

My grandmother was placed here for rehab. We could never get a consistent answer about her care. All that I know is that my grandmother was coherent upon entry and was administered an unnecessary amount of Xanax and Benadryl that I believe was used as a chemical restraint.

Rehab patient's family · 2023☆☆☆☆

The aides and housekeeping staff are stellar! Her room is always clean and the aides are attentive and always available if she needs a bit of help.

Glen Arden Inc shows recurring issues with safety hazards appearing in all three surveys, alongside persistent problems with care planning and food safety standards. The facility has faced deficiencies in accident prevention, meal quality and food handling, resident assessment planning, infection control, and medication management. While all violations were reportedly corrected by the provider, the repeated safety and care planning issues across multiple years suggest ongoing operational challenges that families should discuss during visits.
