The facility failed to ensure the medication administration record (MAR) was accurate for one resident. Specifically, there was no documentation of Cogentin 1mg being administered as ordered from March 8, 2023, through March 31, 2023.

The facility failed to ensure a resident was free from exploitation regarding the use of personal funds. Specifically, a resident's personal funds were used to purchase major appliances for the facility, involving a verbal agreement for a loan that was not properly documented.
