While some families report excellent experiences with the therapy team and specific staff members, there is a consistent pattern of complaints regarding neglect, hygiene, and slow response times. If you are considering this facility, we strongly recommend unannounced visits during evening or weekend hours to observe staffing levels and cleanliness firsthand.

Key Review Excerpts

The Respite Care stay at Fouraker Hills was a pleasant experience for my mother. The Director of Admissions, MN, was very prompt and attentive regarding any questions or concerns that I had during her stay.

Respite care family member · 2024☆☆☆☆

The facility itself is in poor condition—it's dirty, has a strong odor of urine, and is poorly lit, with most lights either off, dim, or not working at all, making the building feel very dark.

Long-term resident's family · 2024☆☆☆☆

If I could give 3.5 stars I think that would be most appropriate. Fouraker Hills seems to have the basic needs met which is perfectly fine. They have decent sized rooms, availability and the staff seem attentive and professional.

6 complaint-triggered

This facility has concerning patterns with families filing 6 complaints that triggered federal inspections, revealing issues with medication management, infection control, and privacy protection. The facility shows recurring problems with quality of care (affecting daily living abilities appears in multiple surveys), resident rights violations (privacy breaches found twice), and safety hazards (accident prevention cited repeatedly). While all deficiencies show correction dates, the persistent nature of similar violations across multiple years and the significant number of family complaints suggest ongoing operational challenges that merit careful consideration during any visit.
