What this means for your family

While many families report positive experiences with the therapy and activity departments, there is a recurring pattern of communication failures and reports of neglect. We strongly recommend visiting in person at different times of the day, including weekends, and speaking directly with the Director of Nursing to ensure your loved one's specific medical needs will be met consistently.

Key Review Excerpts

My sister lived at Four Seasons for 4 years before she passed, in the dementia unit. The staff treated her like family. They were very attentive to her needs and worked with us on every aspect of her care.

Memory care family member · 2021★★★★★

I don’t understand the high Medicare ratings for this place. It was terrible for my mom. The place looks run down and the furniture and beds are cheap and shabby, but we could live with that if she received good care. Unfortunately this was not the case.

Long-term resident's family · 2024☆☆☆☆

My mother has lived at Four Seasons for 4 months now and I expect she will be there for a long while. I am very pleased with the care she gets. She is calm and happy! She has on clean clothing every day and her room is kept very clean.

Families have filed numerous complaints triggering federal inspections, with recurring issues in treatment and care quality, medication management, and accident prevention. The facility shows persistent problems across multiple surveys from 2022-2025, with repeated violations for providing appropriate treatment according to orders, medication errors and labeling issues, and maintaining safe environments free from hazards. While all deficiencies show correction dates, the pattern of ongoing complaints and repeat violations across the same care areas suggests systematic quality concerns that families should investigate thoroughly before placement.
