What this means for your family

Five Oaks has a strong reputation for its admissions process and therapy department, which many families find helpful during transitions. However, due to consistent reports of poor communication and staffing shortages on weekends, we strongly recommend that families plan to visit frequently, especially during off-hours, to ensure their loved one's needs are being met.

Key Review Excerpts

The nurses were ready for my needs and the business conducted for me was efficient with a smile. Ty Jones set up my papers and surely knows her job expertly.

Rehab patient · 2025★★★★★

I'm a current patient here at 5 oaks and 4 star reviews are fake and misleading. There is lack of professionalism, protocols and training ,especially for nights/weekends. The food is atrocious.

Current patient · 2024☆☆☆☆

My mother was a resident at Five Oaks for three years before she passed away. Five Oaks wasn't perfect but none of these facilities are given the circumstances. They always treated my Mom and me with the utmost of respect.

This facility shows recurring safety and care quality concerns across multiple surveys, with families filing complaints about accident prevention and infection control. The most frequent problem areas involve accident hazards and supervision, care planning and resident assessments, and building safety systems like fire sprinklers and emergency exits. While all deficiencies report corrections, the pattern of repeat violations in critical areas like accident prevention suggests persistent challenges in maintaining consistent safety standards.
