The nursing home lied and falsified documentation when state was alerted. My mother fell out of bed on a Sunday, night shift, with no call bell, had 2 black eyes, broken wrist, bruised ribs and sepsis because of neglect and no toileting.

The night staff would put my Dad to bed at 7 and the morning staff would not not get him up until 8:30 - 9 am. 12-14 hours in bed?! He had a decubitus ulcer that turned into osteomyelitis because of poor care!
