The Licensing Program Analyst found that the allegations were unsubstantiated based on the evidence reviewed, including resident documentation and interviews.

This report details a complaint investigation conducted on February 5, 2026, regarding the quantity or quality of food provided to residents. The investigation found the allegation to be UNFOUNDED after reviewing menus, touring the kitchen, and interviewing staff and residents.

The facility underwent an unannounced annual inspection, during which the LPA toured all areas including resident rooms, medication rooms, and common areas. No immediate health, safety, or personal rights violations were observed during the inspection.

The inspection was conducted by an LPA for an annual review. The LPA reviewed 15 resident files and observed that all required paperwork was present in the files. The inspection was incomplete and the LPA plans to return at a later date to finish the annual inspection.

The facility underwent a Case Management visit conducted by an LPA. The analyst interviewed staff and reviewed documentation, including R1's paperwork and hospital discharge records. No deficiencies were cited at the time of this visit, and the facility was determined to be acting appropriately.

The facility was inspected by the Licensing Program Analyst to follow up on a death report for a non-hospice death. The LPA reviewed pertinent documents and spoke with the Executive Director and Resident Care Director. No deficiencies were cited during this visit.
