A relicensure survey was completed on 11/16/23. A deficiency was cited.A change of ownership occurred on 5/3/21. Based on record review, observation and interview, the licensee failed to notify the department of a change in administrator, at least 30 calendar days in advance, affecting eight current residents. Findings include:1. ObservationOn 11/16/23 from 7:30 a.m. to approximately 3:00 p.m., a paper posted to the residence' s wall by the front door read in part: "Owner and Administrator." This was followed by the owner' s name and the owner' s telephone number.2. Interview On 11/16/23 at 7:30 a.m., Staff #1 stated that the owner was not available, but the manager would arrive soon. On 11/16/23 at 8:58 a.m., the manager stated she started training to become the administrator of the residence on 12/1/22, adding that the administrator of record was no longer working at the residence by that time. The manager stated that the owner was the administrator and she worked under her.On 11/16/23 at 1:33 p.m., an electronic message received from a department representative read in part that the licensee had not submitted an application to notify the department of the change in the administrator. She stated that the administrator of record' s name was listed when the residence renewed their license in April 2023.On 11/16/23 at approximately 2:30 p.m., the owner stated that the required paperwork did not go through because she did not pay the required fees.
