Key Review Excerpts

my mom has been there not even a week now and has been sent to the hospital 3 times due to her blood sugar. They have changed her insulin twice and not following the schedule feedings that needs to be done with a brittle diabetic.

Resident's family · 2023☆☆☆☆

I have tried to contact my mother many many many times and nobody will answer the phone. My sister showed up there and had to wait on an employee for 10 minutes in the lobby for them to get done smoking before they helped her.

Resident's family · 2023☆☆☆☆

The smell of urine is strong and hits your right in the face as your go through the locked door. Someone had urinated on the couch and it was still there soaking wet where other residents could walk in and sit right in it.

The facility failed to ensure sufficient staff were available to provide individual feeding assistance. During a lunch meal observation, staff members were observed feeding two residents at the same time, specifically sitting between residents to assist both simultaneously.
