Key Review Excerpts

My grandma has been here since Feb and her baseline was improving and she just graduated from hospice! I truly believe it’s the care and and attention the staff that has turned physical and</em> mental health around!

Long-term resident's family · 2025★★★★★

I brought my 86 yr old mother to Deer Creek for a five day respite (we both had Covid as did several of their residents.) I told them that my mom had dementia and was a fall risk and usually needed help with walking, dressing, toileting etc. I stuck around for a few hours before I went home for some much needed rest and I noticed that no one came in to check on her

Respite care family member · 2025☆☆☆☆

Multiple families have filed complaints triggering deficiencies at this facility, with recurring issues in treatment and care, family notification requirements, and medication management. The facility shows a concerning pattern of repeated violations across multiple surveys, including recent complaints about food safety and pharmacy services as of December 2025. While all deficiencies show correction dates, the persistent nature of complaints and repeat violations across key care areas suggests ongoing quality concerns families should investigate thoroughly.
