Residents were required to use a community bathroom located down a long hallway, increasing fall risks for a resident known to be a high fall risk. Additionally, the facility failed to notify the resident's family of the inoperable bathroom and did not offer to relocate the residents to a functional room.

As a result, the resident was forced to use a community bathroom located far down a long hallway, which required significant physical effort and the use of a call bell for assistance. Additionally, staff failed to follow up on the resident's request to move to a different room.
