What this means for your family

Creekside has a dedicated therapy team and many staff members who are highly praised for their compassion. However, the facility has a pattern of inconsistent response times and communication failures. We strongly recommend visiting during off-hours and asking specifically about their protocols for call-button response and family care-plan meetings.

Key Review Excerpts

The nursing and CNA’s are very kind. The facility, tho older and worn, is kept very clean and mom has never had so much as a cold while living there.

Long-term resident's family · 2025★★★★

The nursing staff does not respond to the patients call button. My brother has had to wait up to 2 hours before the nurse comes to his room. This is totally unexcepible.

Family member · 2024☆☆☆☆

The memory unit is awful, the smell of urine is overwhelming. During today’s visit the roommate - a naked man with a Cather and urine bag attached to his thigh attacked and was choking a resident that wondered into the room.

9 complaint-triggered

This facility shows recurring patterns of fire safety violations and daily care deficiencies, with families filing seven complaint-triggered reports over the review period. The most persistent problems involve fire safety systems, daily care assistance, and resident rights protections. While all violations have reported correction dates, the repeated fire safety issues across multiple surveys and ongoing family complaints about care quality suggest systemic challenges that may require careful evaluation during any visit.
