Resident A was admitted May 1, 2025, but there was no record of weight at admission or in June 2025.
Hot water temperature at the kitchen fixture was 130 degrees Fahrenheit (Repeat Violation).
Upstairs bathroom required repair: missing trim, walls required painting, flooring required repair, and stained shower door; radiator covers were missing.
One bedroom door on the second floor was not affixed to the frame.
Backdoor steps were not equipped with handrails on both sides.
No fire drill conducted during sleeping hours during the 1st quarter of 2025 (Repeat Violation).
