Key Review Excerpts

My brother has been in long term care for five years. I have seen some very crappy care in many facilities. He came to Cornerstone and right away they cleared up his diaper rash that the other facilities couldn’t do, they cleared up his dandruff and they have inspired him to get up out of his and socialize.

Long-term resident's family · 2026★★★★★

I was just discharged from Cornerstone Gardens Rehab. This was my first exposure to Cornerstone and I am singing their praises for the exceptional care I received from all the staff I encountered during my stay.

Rehab patient · 2026★★★★★

The only issue I have is that the weekend staff seems to be lazy. You come in during the week and everything's taken care of but when you come in on the weekend it seems like it is a different crew.

Cornerstone Gardens has a concerning pattern of recurring deficiencies across multiple areas, with families filing complaint reports about infection control and privacy issues. The facility has repeatedly struggled with infection prevention, resident rights protection, food safety, and fire safety systems across four surveys since 2023. While all deficiencies show correction dates, the persistence of similar problems—particularly with infection control appearing three times and resident rights violations twice—suggests ongoing operational challenges that families should carefully consider.
