The facility failed to ensure the implementation of physician orders for a resident regarding the daily application of thromboembolic-deterrent (TED) hose. Record reviews and observations confirmed the resident was not receiving the ordered treatment despite the physician's instructions.

The facility failed to ensure that medication administration records were accurate, specifically regarding the documentation of the medication aide who administered the medications. Discrepancies were noted between the electronic medication administration record (eMAR) and the controlled medication log for Tramadol.

The facility failed to provide necessary bathing assistance and nail care for a resident. Observations revealed the resident had long, dirty fingernails, skin flakes on the scalp, and greasy hair, despite the care plan requiring extensive assistance with these tasks.

The facility failed to ensure proper follow-up for a resident's medical needs. A wound on the resident's left great toe, characterized by redness, inflammation, and yellow drainage, was not reported to the primary care provider or home health immediately.

The facility failed to provide adequate bathing assistance and nail care for a resident. Observations revealed the resident had long, dirty fingernails and unkempt, greasy hair with skin flakes on the scalp. Documentation and interviews indicated that staff had not consistently provided necessary hygiene assistance as required by the resident's care plan.

The facility failed to ensure walls, floors, and ceilings were in good repair. Specific issues included dry rot and unpainted baseboards in bathrooms, rusted floor vents, and buckling floors that caused dining room doors to stick and require force to open.

The facility failed to ensure furniture was kept clean and in good repair. Observations of outdoor porch chairs revealed detached fabric on one chair and a large two-foot tear in the fabric of another, exposing the metal frame.

The facility failed to maintain ceilings in good repair, specifically in resident room #7. Observations revealed large areas of brown staining on the ceiling caused by previous water damage from a roof leak.

The facility was unable to maintain a minimum hot water temperature of 100 degrees Fahrenheit. This deficiency affected the staff bathroom, six resident bathrooms, four resident-room showers, and the laundry room sink.
