The facility failed to maintain hot water temperatures within the required range of 100 to 116 degrees Fahrenheit. Specifically, four out of four sampled fixtures, including bathroom sinks and showers, were found to have temperatures as high as 132 degrees Fahrenheit. Additionally, no caution signs were posted to warn of the high water temperatures.

The facility failed to maintain hot water temperatures between 100°F and 116°F in the common residents' bathroom on the east hall. Temperatures were recorded as high as 126.6°F at the sink and 122.6°F at the shower, with visible steam present.

The facility failed to ensure that 2 of 3 sampled residents were tested for tuberculosis disease upon admission in compliance with required control measures. Specifically, for Resident #1, there was no documentation of a second-step TB test administered since their admission in 2018.

The facility failed to ensure hot water temperatures were maintained between 100 and 116 degrees Fahrenheit. Specifically, temperatures at the sink and shower in the east hall common resident bathroom were measured at 128.6°F and 122.6°F, respectively.

The facility failed to ensure that all residents are tested for tuberculosis upon admission. A review of records revealed that 2 of 3 sampled residents (Resident #1 and #3) were not tested for tuberculosis at the time of admission.
