A certification complaint, prompted by #CO38318 and #CO35384, was completed on 11/19/2024. A deficiency was cited. Based on interview and record review the facility (residence) failed to ensure staff accurately documented medication administration affecting two of two sample members (residents) (#1,#2).Findings include:1. Residence PolicyThe residence' s policy titled, Medication Administration, dated March 2023 read in part: "Documentation of Administration of Medication to residents states medications administered in the ALR will be documented on a medication administration record (MAR). The Administrator, together with the qualified medication administration person (QMAP) Supervisor will be responsible for ensuring the accuracy and proper use of the MAR." All medications will be listed in the resident ' s MAR, the date and time administered, and "any time a medication is refused or not administered for any reason, this will be documented in the MAR."2. Resident #1 was admitted to the residence on 04/02/23 with diagnoses consisting of benign prostatic hyperplasia hypoxemia, type two diabetes mellitus, hyperlipidemia, and unspecified other arthritis.a. Novalog Flexpen A written practitioner' s order, dated 10/11/24, directed the residence to administer Novalog Flexpin 8 units three times daily before meals. However, the October 2024 MAR read the medication was not administered on 10/13/24 at 12:00 p.m. and 5:00 p.m. as the resident was not at the residence.b. Insulin Glargine A written practitioner' s order, dated 10/11/24, directed the residence to administer Insulin Glargine 30 units daily at bedtime. However, the October 2024 MAR read resident refused and was out of the residence on 10/13/24.c. Midodrine HCL A written practitioner' s order, dated 9/18/24, directed the residence to administer Midodrine HCL 5 mg three times daily. However, the October 2024 MAR read the medication was not administered on 10/13/24 during the noon and evening doses as the resident was not in the residence.d.
