Water temperatures were not maintained between 105 and 120 degrees Fahrenheit in three resident rooms, causing resident dissatisfaction and placing residents at risk of discomfort and decreased quality of life.

Deficiencies regarding water supply and temperature requirements were previously cited and have been corrected.

The facility failed to obtain medications in a timely manner for 3 out of 8 sampled residents (Residents 1, 3, and 4) due to pharmacy re-ordering and re-filling issues, resulting in multiple missed doses for various prescribed medications.

The facility failed to include appropriate behavioral interventions in the negotiated service agreement for a resident with a known history of physical and verbal aggression toward others.

Facility failed to obtain prescribed medications in a correct and timely manner for 2 of 3 sampled residents, resulting in missed doses for multiple medications including magnesium oxide, aripiprazole, famotidine, hydroxyzine, tizanidine, mirtazapine, bupropion, trazodone, clonidine, guaifenesin, vitamins, gabapentin, buspirone, and atorvastatin.
