What this means for your family

While some families report positive experiences with the therapy department and specific staff members, there is a recurring pattern of severe neglect, particularly regarding hygiene and response times. If you choose this facility, we strongly recommend frequent, unannounced visits to monitor your loved one's care and to ensure that basic needs are being met consistently.

Key Review Excerpts

My mother arrived for what was supposed to be a temporary stay, to receive therapy to have a better quality of life. Needless to say the physical therapy was subpar, she never got any occupational therapy, and they don’t even have a full time speech therapist; instead of improving, she ended in much worse shape.

Rehab patient's family · 2024☆☆☆☆

My very first visit i found mom on the floor with her arm trapped under her, on the fall mattress. It took over 45 minutes to get help to get her up.

Long-term resident's family · 2025☆☆☆☆

I have seen excellent teamwork with both floor staff and managers. The staff is friendly when I pass them in the hallways. I see people going out of their way just to do sweet things for the reidents.

Clean record — no deficiencies

This facility has a concerning pattern of family complaints that triggered 6 deficiencies, including serious issues with accident prevention, nursing staff competency, and feeding tube protocols. The main problem areas involve safety supervision, fire safety systems, and resident rights communication. While the facility has corrected most issues, the repeat safety violations and multiple family-filed reports suggest ongoing care quality concerns that warrant careful evaluation during a visit.
