Key Review Excerpts

The only positive thing about this place is the nurses on the Fulton Unit are absolutely amazing as well as the CNAs.

Family member · 2021★★☆☆☆

Some of the food you can’t even recognize, and instead of doing therapy with your loved one they will recommend you stop therapy and put them on hospice so they can get the next paying patient in the door!

Family member · 2021★★☆☆☆

Such an amazing nursing facility with staff who are loving and dedicated in caring for their patients!

2deficiencies
Well below state avg (7.6)

Chase Center shows a pattern of safety and care planning issues across three years, with fire safety systems (sprinkler maintenance) cited repeatedly in 2023 and 2025, plus recurring problems with resident assessment and care planning. The facility has corrected all identified deficiencies, but the repeat fire safety violations and spread of issues across multiple care areas suggest ongoing operational challenges that families should discuss during visits.
