What this means for your family

This facility offers a clean environment and strong rehabilitation services, which are great for recovery. However, families should be aware of significant communication challenges; I recommend establishing a direct line of contact with a specific nurse or social worker early on to bypass the unresponsive main office.

Charleston Healthcare Center receives polarized feedback, with recent reviews highlighting a clean, modern facility and a strong rehabilitation team, while others report significant concerns regarding administrative responsiveness and patient safety. Families frequently praise the physical environment and individual staff members, but multiple reports of unreturned phone calls and communication breakdowns regarding patient care are recurring issues.

Key Review Excerpts

The facilities are always so clean and the staff is so friendly and helpful!

Visitor · 2024★★★★

The therapists are caring and compassionate. Very nice and clean facility. We were very impressed. The staff and nurses and help go out of their way to assist you in any way.

Rehab patient's spouse · 2020☆☆☆☆

It is beyond frustrating attempting to call and check on a patient. Literally, no one answers the phone. I do not understand this because when I go to visit the patient, multiple employees are sitting the the nurses station.

This facility shows concerning patterns with families repeatedly filing complaints that triggered 62 of 113 total deficiencies. The most persistent issues involve safety hazards, food service problems, and infection control failures that recur across multiple surveys from 2021 through 2024. While the facility corrects violations after citations, the ongoing cycle of family complaints about basic care and safety suggests systemic problems that families should carefully evaluate before choosing this facility.
